Group Therapy Fees:

I accept credit, debit, cash, or check. All fees are due at time of session. Movement+ Mindfulness classes require payment at time of booking.
HSA funds may be used for specific services which include individual and group therapy. Prior discussion with clinician is required so proper documentation can be ensured for your records.

Yes. Therapy should be accessible for all. I offer a sliding scale for those who are interested in individual therapy. Note, I may offer group as an alternative to individual as the group fee of $40 is less than the lowest sliding scale fee. I am happy to work with you within your specific financial parameters. Group therapy is a highly effective method of treatment. Part of what makes it effective is ensuring all members of the group are committed to the group process, are appropriate for the group they are wishing to join, and will be an overall good fit with the other members. It also gives you the opportunity to ask any questions you may have, see the therapy space prior to meeting everyone else, and to meet the clinician prior to starting group.

Group Therapy involves multiple people and therefore an agreement on behavior is appropriate when a group is formed. It is protective to members and the treatment process.
Giving appropriate time and space to the group process is why group therapy seems to be so intense and has “rules”. This gives members of the group time to settle in with each other and experience the benefits of group work. Joining a group for the first time requires an initial 8 week commitment. After your first 8 week commitment you are free to leave group at any time with proper goodbyes.
Each group will operate with clear goals and an agreement for group behavior. 1. Be in the here and now. 2. Be on time. 3. Keep the identities and specifics about your group mates confidential. 4. Pay your fees. 5. No secrets.

It is okay to keep living your life while in group. Living means there will usually be instances when we will need to miss a session.
Please be courteous and let your group and clinician know ahead of time when you will be gone. There are no refunds for missed sessions.

Practice Policies and Consent to Treatment
1. Introduction:
At Rhizome Mental Health Services, LLC (Rhizome) our mission is to provide high-quality mental health services through individual and group therapy as well as digital courses and programs.
2. Services Provided:
Rhizome provides behavioral health services for people experiencing distress in their lives in an office setting. Services may include, but are not limited to, group therapy (processing and skill building), online virtual programs, individual consultations, individual therapy, supervision for Limited License Master Social Workers, and community education seminars. Rhizome retains the right to add or discontinue services at any time.
3. Appointment Scheduling & Cancellations:
All clinical sessions are scheduled by appointment only. Clinical sessions include group therapy, group skill building/psychoeducation, individual therapy, and individual consultations. Clinical clients must be in the state of Michigan at the time of service. Community sessions, including movement classes and community education seminars, require registration via online booking or by calling or emailing. These community sessions do not require pre-approval.
Any participation in group therapy requires a 20-minute individual consultation prior to the start of the first group therapy session where a potential client will be assessed to ensure a good fit with the group members. This session is at no cost to the potential participant. If at the end of the consultation the group therapy sessions are determined to be appropriate for the potential participant, in our sole discretion, then the potential participant will be asked to sign paperwork including agreeing to the group social contract, and review practice policies. This individual consultation also gives a potential participant a chance to ask any questions they may have.
Online booking is available for community classes, group skill building series, individual consultations, and returning individual therapy clients. Group therapy is not available for online booking as group therapy requires a commitment to a series of sessions. If you agree to attend group therapy, then you are committing to attend the entire series.
Cancellations are allowed if more than 24 hours prior to session. Late-cancellations (less than 24-hours prior) or no-shows for any service, including group therapy, will incur the cost of the service and be billed directly to the payment method on file. Please do not come to a session physically ill. Rhizome reserves the right to refuse services to anyone that may have a contagious physical illness or terminate services with clients who have three late cancellations or no-shows.
4. Telehealth Policy:
Limited services are offered through telehealth. These services include individual therapy, community education series, certain movement classes, and some group skill building sessions. All telehealth sessions are conducted through secure platforms.
It is your responsibility to ensure a private, quiet environment for telehealth appointments. You are prohibited from recording, sharing, saving images/screen shots, or including others off camera. Rhizome reserves the right to discontinue telehealth services with any client who violates these stipulations or otherwise is not actively participating in the session while engaged in telehealth. If you are experiencing technical issues, then such issues must be reported promptly to the therapist.
5. Confidentiality:
As required by law, all client information is confidential. Guidance on HIPAA regulations is available on our website and by request. There are exceptions to confidentiality obligations that include, without limitation, if you pose a risk of harm to yourself or others, court orders, or abuse/neglect of a minor or vulnerable adult.
Participants of clinical groups with be asked to agree to the terms of the group which includes protecting the privacy of other group members by not discussing, identifying, or broadcasting (via social media or other online outlets) information discussed within the group setting.
6. Fees & Payment:
Fees are due at the time of service unless prior arrangements are made. Accepted payment methods include credit/debit cards, Health Savings Account (HSA) funds for eligible services, checks, and cash. HSA funds may only be used for group therapy and psychoeducation. If you intend to use HAS funds, then you must alert the provider at time of initial consultation or booking so proper documentation and paperwork can be executed.
7. Emergency Contact:
Rhizome provides mental health services. Our services are not appropriate for crisis situations or if you are suffering physical distress. If you or someone you know is experiencing a crisis and needs immediate medical assistance, please call 911 or go to the nearest urgent care provider.
Part of onboarding paperwork includes providing a name and phone number in case of emergencies. This emergency contact will only be contacted in the event of an emergency, as determined by the provider, while you are receiving services either at the office or via telehealth services. Examples include a fire in the building or an injury during movement class that requires help getting home or advance medical care. Your emergency contact does not have a right to ask for your protected health information and will not be provided any information if they inquire unless you consent to provision of such information.
8. Client Rights & Responsibilities:
Clients have the right to confidentiality, respectful treatment, and informed consent.
Clients are responsible for their behavior while receiving services. You are expected to be on time for scheduled sessions and participate at the agreed upon level of engagement for the specific type of session, course, or class. You are expected to be respectful of others’ beliefs and values which fosters an environment of openness and care for each person as an individual. Rhizome reserves the right to end services without notice for clients who do not adhere to these stated responsibilities.
9. Social Media
Rhizome does have a social media presence and as a client you have the choice to engage with or on social media or not. Rhizome will not follow or add current or former clients as friends. Rhizome will also not repost any client’s posts or stories that tag the business. The clinician has a personal presence on social media and will not accept friend requests, direct contact requests, or follow/friend request current or former clients. 10. Acknowledgement and Consent to Treatment:
I have read and accept this agreement and herewith consent to counseling/psychotherapy with Rhizome.
